Who are we?

Sefton Carers Centre Hospital Discharge Link Workers work with carers of people who are coming out of hospital. We can provide you with information about support and services for you and the person you care for when they are discharged from hospital.

Hospital Discharge

As a carer of someone who is due to be discharged from hospital, you should be consulted about:

Discharge plan for the person you look after

The date the person you care for will be discharged and what, if any, services they will receive

Practical information regarding issues such as transport, medication, equipment, care at home

Your own needs as a carer

What help can we provide?

We know that caring for someone can be a demanding task at the best of times. When the person you care for is in hospital, it can become even more stressful, and it may completely change your caring role or you may even be becoming a carer for the first time. We are here to help you through this difficult time.

As Hospital Discharge Link Workers we can:

Provide you with contact details of the person you need to speak to in the hospital about the person you care for

Complete a Carers Needs Assessment to take your own needs into account

Provide information about other support and services that are offered by Sefton Carers Centre.

Provide personal support for you as a carer at Discharge Planning Meetings if necessary.
